REQUEST AN ASSESSMENT QUOTE

Complete section A on the HAIS assessment booking form and send to rachel@thejusticeintermediary.co.uk.

If I am able to accept the case, I will complete part B and the quote section and return it to you.

You complete section C and send to either LAA or HMCTS as appropriate.

Once you have funding confirmed, forward it to me and we can arrange the assessment appointment.

REQUEST AN ATTENDANCE QUOTE

Complete section A on the HAIS attendance booking form and send to rachel@thejusticeintermediary.co.uk.
If I am able to accept the case, I will complete part B return it to you to send to the court (venue on the form!) for them to authorise.
Please help me by reminding the court to fill in section C including a purchase order number.
ONLY ONCE I HAVE A COMPLETED FORM WILL I BE ABLE TO RESERVE DATES FOR ANY ATTENDANCES.

IMPORTANT INFORMATION
